1) 10 specimen of the work to be applied as copyright. (4 sets of floppy discs in case of computerprogram).
2)documentary proof of publication in India, alongwith details of Name,Nationality, age, address of the publisher and date of Firstpublication if any.
3)General Power of Attorneyduly signed by the applicant if the applicantis other than a natural person partner or one of the Directors of thecompany as the case may be.
4) In case of computer program brief description of software in 200 words.
5) A copy of Memorandum & Articles of Association or copy of Partnership Deed as the case may be.
6) Author's Name, Nationality, age, address and if the author is deceased, date of death.
7) No objection Certificate from the author.
8)If the artwork is capable of being used in respect of goods, aCertificate from the Registrar of Trade Marks to the effect that Nosuch Trade Mark is registered or application for registration Is madeby a person other than the applicant.

1) Name, Nationality, age, address of the publisher, who published Subsequently.
2) Details of various rights and their holders.
3) Possession of Original Work.
